Sample Submission Forms
We are excited to announce we have launched a new online submission system. Please be patient while we work out the kinks. An electronic copy will be sent to use once submission is received. Please print copy of the submission (landscape orientation; all on one page) and include it with your samples.
Make sure your sample names match what is written on the tube. A major change is that sample names must be between 2 and 12 characters, uppercase letters ONLY, numbers, and underscores.
